Breast Pain Evaluation in Dhaka Bangladesh
Breast pain evaluation is a medical assessment that identifies whether tenderness, aching, burning or heaviness is coming from the breast, chest wall, hormones, infection or another condition. Dr. Reshma Aziz offers private consultations for new, persistent, one-sided or recurring pain. Breast pain alone is rarely caused by cancer, but pain with a lump, skin change, discharge, fever or worsening swelling needs assessment.

What Causes Breast Pain?
Breast pain, or mastalgia, has several causes. Cyclical pain follows hormonal changes and often affects both breasts before a period. Non-cyclical pain may relate to a cyst, fibroadenoma, infection, abscess, duct change, injury, pregnancy, breastfeeding, medicine or surgery. Pain may instead begin in the chest wall. Muscle strain, rib or cartilage inflammation, posture and bra pressure can feel similar. Examination helps identify the source.
- Cyclical Breast Pain
Often linked to hormonal changes and commonly affects both breasts before a period.
- Non-Cyclical or Chest Wall Pain
May be caused by cysts, infection, injury, breastfeeding, medicines, muscle strain, rib/cartilage inflammation, posture, or bra pressure.
When Should Breast Pain Be Checked?
Book when pain is new, one-sided, focal, worsening, waking you or limiting activity. Seek assessment with a lump, thickening, underarm swelling, nipple discharge, new inversion, dimpling or skin change. Get prompt help for a hot, swollen or rapidly reddening breast with fever. Severe illness, breathing difficulty, fainting or crushing chest pain requires emergency care.

Cyclical Breast Pain
Tenderness often affects both breasts and becomes stronger before a period.
Non-Cyclical Breast Pain
Pain has no monthly pattern and may remain in one area.
Focal or One-Sided Pain
Persistent pain in one area may require age-appropriate imaging.
Pregnancy or Breastfeeding Pain
Engorgement, blocked ducts, mastitis, abscess or galactocele may cause pain.
Pain With a Lump or Discharge
Pain with a mass, discharge or skin change needs breast assessment.
Chest-Wall Pain
Pain reproduced by movement or rib pressure may come from outside the breast.
- Breast Pain Does Not Confirm or Exclude Cancer
Most breast pain is not caused by cancer. However, pain cannot classify a lump or visible change. Avoid repeated checking, unprescribed antibiotics and delayed assessment.
Step-by-Step Patient Journey
Symptom History
Dr. Aziz asks about location, onset, menstrual pattern and triggers. Pregnancy, breastfeeding, injury, medicines, hormone use, surgery and family history matter. A diary with dates and pain scores from 0 to 10 can reveal patterns.
Respectful Examination
With consent, the breasts, nipples, underarms and chest wall may be examined. Clinical lighting, an examination couch, gloves, gown or drape and privacy screen support dignity. A female chaperone can be requested.
Decide Whether Imaging Is Needed
Imaging is not automatic for diffuse or cyclical pain with a normal examination. Ultrasound uses a transducer and gel to examine focal pain or a lump. Diagnostic mammography uses breast X-rays and compression views. Test choice depends on age, pregnancy and findings.
Further Investigation When Indicated
A suspicious finding may require fine-needle aspiration or core biopsy using antiseptic, local anaesthetic and image guidance when needed. Labelled samples go to pathology. Pain alone does not routinely require biopsy.
Personalised Management and Follow-Up
Management may include reassurance, a supportive bra, posture changes, suitable pain medicine and medication review. Infection may require antibiotics, while an abscess may need drainage. Do not change prescriptions without advice.
How Do Cyclical, Non-Cyclical and Chest-Wall Pain Differ?
| Factor | Cyclical Breast Pain | Non-Cyclical Breast Pain | Chest-Wall or Referred Pain |
|---|---|---|---|
| Pattern | Changes with menstrual cycle | No regular monthly pattern | Often changes with movement or pressure |
| Usual Location | Commonly both breasts | May affect one breast or a focal area | Rib, muscle or cartilage beneath or beside breast |
| Common Descriptions | Fullness, heaviness or tenderness | Burning, aching or sharp pain | Local tenderness or pain with upper-body movement |
| Initial Assessment | History, examination and symptom diary | History and focused examination | Breast and chest-wall examination |
| Imaging | Often unnecessary when diffuse, cyclical and examination is normal | Considered for persistent focal pain or abnormal findings | Guided by clinical findings, not automatically required |
What Breast Pain Treatment May Be Recommended?
Treatment follows the cause. A supportive bra and removing a clear trigger may help. A clinician may advise a pain reliever or topical anti-inflammatory after checking allergies, pregnancy, kidney problems, stomach ulcers and blood thinners. Hormonal contraception or menopausal therapy should not be changed without the prescriber. Antibiotics are reserved for likely infection. Aspiration, drainage, biopsy or surgery requires a clinical reason.

Frequently Asked Questions About Breast Pain Evaluation
Get clear answers to common questions about breast pain, evaluation, imaging and when medical assessment may be appropriate.
No. Breast pain alone is rarely caused by cancer. However, pain accompanied by a new lump, skin or nipple changes, nipple discharge, persistent focal pain or other breast changes should be assessed by a healthcare professional.
No. The need for imaging depends on factors such as the type and location of pain, age, pregnancy or breastfeeding status, examination findings and the presence of other symptoms. A clinician can determine whether ultrasound, mammography or another assessment is appropriate.
Breast pain that occurs on most days for more than two weeks, continues to worsen or interferes with daily activities should be reviewed by a healthcare professional. Persistent or localized pain may require examination to identify the underlying cause.
Yes. Muscle strain, inflammation around the ribs or cartilage, posture and other chest-wall problems can produce pain that feels like it is coming from the breast. Clinical examination can help distinguish breast-related pain from chest-wall pain.
Not without medical advice. Painful swelling can have different causes, including infection or an abscess, and antibiotics are not appropriate for every cause. An abscess may require clinical assessment, imaging and, in some cases, drainage.
The current cost depends on the consultation, examination and any imaging or procedures that may be required. Contact the clinic for the current consultation fee and ask whether ultrasound, mammography or other tests are billed separately.
